MMI Questions and Feedback
MMI Question #1
Category: MMI Roleplay
Question: You are a 3rd-year medical student working under the supervision of a senior. You are observing a patient who is suffering from end-stage cancer. These patients do not recover much, and death is almost inevitable in such cases. Treatment in such cases can only delay and prolong death. The doctor has advised the treatment, but the patient has asked to stop the treatment against the doctor's advice. The patient's daughter is here to meet with you because the senior doctor is busy with an emergency call. She says that the senior doctor should continue with the treatment during the meeting and not do what the patient has asked. Explain how to respond to the situation?
Feedback:
Average Response:
Getting into the situation and explaining the solution of the issue.
Inability to understand what the next of kin wants as a result
Promising things and making statements that are above the role described.
Excellent Response:
The daughter's emotions should be kept in mind and given more importance irrespective of the patient's illness, treatment, and diagnosis. One thing that needs to be kept in mind is that the daughter accepts the imminent loss of her father and thus goes through a rough emotional patch.
Using empathy as a tool, ask why she wants this- try to find further information to produce more effective interpersonal responses.
Try to convey that you are a student and it is not the student's duty to make such decisions.
Explain different scenarios that:
- Even if you convince the patient and consultant to take the treatment, the patient might go through an emotional breakdown, and the result would be the same ( the patient's death).
- The patient is autonomous, and he can make his own important decisions.
- Describe that it is not your duty to decide as you are a medical student.
- Convince her to do a meeting with a social or palliative care worker.
- Clarify your stance that your primary duty is the patient's welfare, and you are his well-wisher.
MMI Question #14
Category: MMI Ethics
Question: A 34 year old doctor named James Vince is an active Medical Practitioner. He takes part in many different healthcare awareness campaigns and was asked to participate in a campaign directed against the issue of same-gender marriage. He accepted the proposal and made an enthusiastic video directed against the same-gender marriage issue. The footage produced an outrageous response from the general public. There were protests and demonstrations against the tape. They stated that the doctor is biased and not worthy of practising medicine. The people thus put forth the demand of invalidating the doctor's license to practice. What are the ethical issues that need to be addressed in the above scenario?
Feedback:
Average Response:
The doctor should not participate in controversy.
Moreover, when speaking about topics, he should that what he is saying is his own opinion/point of view.
Exellent Response:
There are some points in the scenario that needs to be addressed, these are:
1. The doctor should not take part in awareness videos (on topics) that are sensitive in nature. The doctor should be taught that it is not a part of their job. The doctor should know what sort of topic is sensitive and thus speak smartly. Awareness regarding sensitive issues should be made as well
2. If a doctor wants to speak on these types of topics, he should say that it is his own opinion and that the people have the right to disagree with him.
3. People should be addressed that the doctor has his own opinion and has the right to speak for. The people should also be taught that there is no need to cancel the doctor's license just based on one incident.
MMI Question #15
Category: MMI Teamwork
Question: The busy routine of a medical practitioner makes it difficult to find the time to fit everything into the standard working day. Many minor essential functions for a large number of patients were not being conducted by doctors in a timely fashion. To address this problem nurses and other paramedics were trained to help the doctor perform many tasks. What are the advantages and disadvantages of nurses in the healthcare system?
Feedback:
Average Response:
The advantage of having nurses in the healthcare system is that they can help the doctor carry out simple tasks and thus increase the speed of delivery of the healthcare system.
The disadvantage is that a slight miscommunication can lead to disastrous effects such as the loss of lives.
Exellent Response:
There are many advantages and disadvantages of having nurses in the system.
Advantages:
• The significant advantage of having nurses in the health care system is that the doctor can perform important major tasks and leave the less critical functions to the nurses. This helps the doctor care for a considerable number of patients in a minimal amount of time.
• They can and do educate the people around them. This helps society to be more literate and understandable towards healthcare. Like in the modern-day pandemic, the nurses provide medical assistance by educating the people about the preventive measures. This helps society to become healthy and disease-free.
• The other advantage is the constant medical vigilance. This is then reported to the physician.
Disadvantages:
The disadvantages of having are more minor as compared to the advantages
• The major drawback is that the minor communication error can lead to heavy damage, including the patient's death.
• The nurses are not highly qualified. So, challenging and critical patients can not be simply handed over to nurses for health care.
MMI Question #17
Category: MMI Teamwork
Question: Many tasks are performed as a group in medical school. The research project is one of the prime examples of such a task where one person alone is not sufficient to do the work. Take the scenario where you are tasked with a research project together with 5 other colleagues. The work needs to be completed within a month but the progress is slow because one of your colleagues is not pulling their weight. What should you do?
Feedback:
Average Response:
I would go to the person not working, and I'll ask him, "why are you not working on the project"? If he provides a solid reason, I'll help him out. Otherwise, I'll report the issue to the supervisor.
Excellent Response:
The points that need consideration in the above scenario are:
• I would go to the culprit; ask him about the reason for not working on the project. If there is a genuine issue that exists. I'll try to find potential solutions to the issue. Then, I'll give a solution to the problem.
• If the person is just procrastinating then, I'll counsel him to work on the project. I will describe the importance of the work and the potential learning outcomes for the person. Hopefully, He'll be ready to work on the project.
• If the supervisor/leader asks me to do the additional work. I will definitely do the job, but I'll tell him about my suggestion of talking on. I'll also try to convince the leader because this task requires teamwork, and the person will definitely learn something from it.
MMI Question #18
Category: MMI Teamwork
Question: In different phases of your medical career, you will be asked to work with other people in a team. What are the qualities of an individual to make them successful within a team?
Feedback:
Average Response:
The good qualities in a team member are politeness and punctuality. If the people are polite to others and are punctual on their tasks, the team will be successful.
Excellent Response:
There are many qualities of a successful person in a successful team. Some of the traits require that the individual should:
• Be a good listener
• Be a good analyzer of himself and the task
• Be a good communicator, and he should be able to give and take clear responses and instructions.
• Be willing to work with others wholeheartedly.
To achieve the traits described above, the person should be honest, professional, empathetic, flexible, and understanding of others. I've demonstrated the features by playing soccer and hockey at my school and college.
MMI Question #20
Category: MMI NHS
Question: In different countries there are different models in which healthcare is provided. For instance, in the USA there is a private model where health insurance or the individual pay for the cost of care. What model is used in the UK and which organization is the major stakeholder for providing healthcare services in the UK? Briefly describe the organization's purpose, responsibility, and role.
Feedback:
Average Response:
The primary organization responsible for monitoring health activities in the UK is the NHS. The NHS stands for National health system. It is a healthcare regulatory authority working in the UK.
Excellent Response:
Many organizations are running different medical activities and campaigns; the prominent name among them is NHS. NHS stands for the national health system in the UK. It is a regulatory authority that was established in the year 1948. It was established to provide healthcare services to the people and is funded by the government. It also receives a handsome amount of donations from the general public as well. It operates in 3 different countries, namely Scotland, England and wales.
MMI Question #21
Category: MMI Hot Topics
Question: The NHS is the primary healthcare-providing authority in the United Kingdom. As a central regulatory authority, its advice is accepted widely and is highly reputable. COVID-19 has been a significant health issue for the past one and and a half years. One initiative launched by the NHS to help mitigate the risks of COVID is the "COVID pass"? How and when can you get the pass?
Feedback:
Average Response:
This is a standard pass that is issued by the NHS upon getting fully vaccinated against COVID-19
Excellent Response:
The COVID pass shows that you are currently not a carrier of the disease or fully vaccinated. There are only three conditions that can permit you to get the COVID pass. These are as follows:
• You have received the total dosage of vaccines (maybe 1 in some and 2 in the others) currently being used in the UK.
• You have a negative PCR or a test named rapid lateral flow test negative in the past 48 hours or less.
• A positive PCR in the last 6 months.
MMI Question #22
Category: MMI Hot Topics
Question: COVID-19 has dramatically affected many people's lives and has adversely impacted people's daily routines in the UK. Each individual has suffered a loss either in the form of their loved ones or in the form of money (i.e. financial loss). There is currently no cure for the deadly disease. The only solution to the problem, as advised by top researchers and health authorities is prevention. The preventive measures include sanitizing the hands, social distancing, and wearing masks. Vaccines are also a potent part of disease protection and are being advised by the health authorities. What vaccines are available in the UK? Who are the eligible people? Use the NHS guidelines to describe the current situation.
Feedback:
Average Response:
The vaccines that are available in the UK are Moderna, Astra Zeneca, and Pfizer. The possible side effects include headache and fever, muscle fatigue, and tiredness may also be reported. Anyone above 16 is eligible for vaccination.

• There are currently three vaccines available in the country and being used, and one will be made available later this year. The available vaccines are Moderna, Astra Zeneca, Pfizer and Janssen vaccine will be made available later this year.
• The eligible candidates include those above 16 years of age, and children aged 12-15 can also receive the vaccines if they are at high risk.
• The adverse effects are:
1. Dizziness and fatigue
2. Headache
3. Pain at the injection site
4. Fever
5. Allergic reactions
6. Blood clotting (very rare)
MMI Question #25

Question: The coronavirus is a highly transmissible disease. Almost everyone among us is at risk of contracting the coronavirus. Lab and front-line workers are at particularly high risk. Assess if pregnant women are at a greater risk of getting the infection? If yes, then explain why?
Feedback:
Average Response:
There is no relation between pregnant women and increased risk of contacting the virus.
Excellent Response:
There is no sufficient evidence to say anything yet. The National Health Service has given the following guidelines:
The thought process is that pregnant ladies might be at a moderately increased risk of contacting the virus. This thought process has not been verified and validated but has been proposed because pregnant females are more prone to get the common cold and flu. The risk of getting a severe infection is more significant in the later pregnancy (3rd trimester). There has been some evidence that there is an increased risk of having a stillbirth, though there is less evidence to support this theory. Ethnic minorities have also been at increased risk for severe infection with COVID-19 during their pregnancies.
The important thing is that preventive measures should be taken to avoid the disease. You can get the vaccines during pregnancy as they are pretty safe and have no teratogenic effects.
MMI Question #26
Category: MMI Hot Topics
Question: The coronavirus has been a significant issue in the world for the past one and a half years. The viral disease has no cure as of yet. The best possible way to manage the coronavirus is to avoid getting the virus in the first place. Many preventive measures are available, and they are the only hope to cope with this problem. If we can eliminate the transmission of the disease, it can help us get over it. To achieve this we need to identify the individuals carrying the disease. What are the tests that are available in the UK that help identify and screen the disease?
Feedback:
Average Response:
Many tests are available. These include PCR tests of the swab sample from the nose and antibody test etc.
Excellent Response:
There are many tests available in the UK that help confirm COVID as the diagnosis. The type of test that is being conducted depends upon the condition of the patient. The two standard tests are:
Rapid lateral flow test: The test is usually designed for screening asymptomatic patients. Many patients with COVID can go undetected; this test is for rapid screening of such patients.
PCR test: this test is conducted to confirm the diagnosis in symptomatic patients. It is also done for patients who have had a positive rapid lateral flow test.

Question: During a 2nd year assessment at medical school you are presented with a graphical image of people working together to get a pyramid constructed. Describe the importance of working as a team, how teamwork is crucial and essential in the medical field and demonstrate a time when you've been a successful member of a team.
Feedback:
Average response:
You start describing your role as a class representative and describe that you led the class very well.
Excellent Response:
I've worked on many teams throughout my career, and I have seen the ups and downs that come with it as well.
I've been a part of the medical society at my school; we worked together to create mini awareness campaigns, different posters, and medical-related projects. The team did well, and at times, the team had to face hard times as well. I was just a member at the start then, and there came a time when I was the senior-most among all the members so, I took charge of the society. Through these experiences, I learned that proper communication is the best tool for the team's success.
Keeping in view the importance of teamwork, I joined my county's cricket team and played with different players. This experience was also fruitful, and I learned a lot. These teams have taught many lessons about teamwork.
MMI Question #33
Category: MMI Roleplay
Question: You are a 4th-year medical student, and during your emergency rotations, you come across a case in which a patient, a 30-year female, is moaning in bed; you go and ask the doctor to attend to the patient but the doctor refuses because he says she has a history of getting admitted to the hospital with minor issues. You are surprised by the harsh and careless attitude of the doctor towards the patient. What would be your next steps to deal with the issue?
Feedback:
Average response:
I would report the doctor to the seniors and the hospital management because the doctor is biased and he is not paying heed to the active complaints of the patient.
Excellent Response:
There are two aspects of the scenarios
The doctor is entirely judgmental, and he is completely ignoring the patient\'s complaints; this is a medical crime and medical negligence. The doctor could be potentially missing many serious complaints of the patients, and it could even result in the patient\'s death.
It might show the doctor\'s professionalism and could potentially lead to the failure of his career and even termination of his license.
The other side is that the doctor might have had bitter experiences with the particular patient, and now seeing the same patient has produced the same sort of response from the doctor. So, the doctor is worried about the potential burden she is placing o the hospital. At this point, the doctor might be right.
MMI Question #34
Category: MMI Ethics
Question: You are a first-year medical student, and you are preparing for your final assessment exams. There is just one week before the start of your exams, and you need to complete and submit an assignment on a pervasive topic. You had discussed it with your cousin, who happens to be a senior at the same school. He then sends you a document that contains the assignments completed by the previous students. What needs to be done here?
Feedback:
Average response:
I would refuse to get help from the provided document as its immoral and unfair means of doing the task would harm my career, and I could get reprimanded. I would also advise my cousin that he should not share his work.
Excellent Response:
Several issues are there in this case scenario. There is an issue with what my cousin has done, and there are also issues with using others' work for the completion and submission of my work. I would try to arrange a meeting with my cousin as early as possible. I'll try to explain to him that his intentions of helping me with my work are good, but several problems exist in this case also. These problems are as follows:
The use of aids for preparing my assignments would hamper my experience. It then would lead to losing valuable experience that might impede my medical professional skills.
If I can explain my point of view to my cousin in the correct manner, he might understand and take corrective measures. If, however, I fail to convince him, I'll talk to my supervisor and friends about it.
In addition, I would like to ask if the assignments of the previous year's students were distributed after their permission or not. Are there any copy-right issues that would result in disciplinary action against them as well?
Suppose the feedback from the previous year's students suggests that their assignment was not distributed with their permission. I would advise them to check what happens to their work and where it goes.

Question: The medical profession requires a variety of different skills to be mastered e.g. teamwork, anatomy, research, analysis, writing, and communication are some prime examples of the skills required in the medical profession. Can you think of any extra-curricular/nonscientific interests that can help improve your skills as a doctor?
Feedback:
Average Response: I think that only simple medical training and other medical-related activities can help anyone improve as a doctor. Excellent Response: The medical field requires anyone, including me, to be versatile and well versed with many skills. The interaction with patients often needs me to be at the doctor's level and understand what the patient demands and requires. Many patients are simple and not able to understand the complexity of their disease conditions. Thus, communication becomes an essential tool to be an effective and good doctor in such case scenarios. I have thought about it several times, and for this, I've started playing cricket. Playing cricket at the county level has enabled me to interact with other teams and players from different regions. This has immensely improved my communication skills and general approach. I believe that these sorts of activities can help me and other doctors grow in their profession.

Question: A graphics designer can earn a good salary, present useful information effectively, and provide visuals of their work. In similar terms explain what sort of benefits and outcomes you expect to get out of the medical profession?
Feedback:
Average Response: I want to achieve a good social status in society. I also want to be recognized at a higher level, such as a government or international level, for my capabilities and services. I would also like to have an excellent financial status out of this career. Excellent Response: The essential motivation and inspiration that drove me towards medicine was seeing my cousin recognized for his work. The recognition was not only from the officials but also from the patients themselves. I want to become a specialist serve people to the best of my abilities. The main goal and target that I want to achieve during my career are opening a Non-government organization (NGO). The purpose of the organization would be to serve ailing humanity. I want to eradicate the deaths in the world happening through malnutrition. I like the NGO to go on a global scale and do the world as a whole. I want to travel to the countries most affected by malnutrition and help them get rid of this problem. I will raise awareness, and I'll train people to diagnose malnutrition. So, most people may eliminate the problem when it starts in the first place. It is the outcome that I want out of my medical career.
MMI Question #37

Question: During a mid-term examination of a 2nd-year medical student, the examiner tells him that a career is an occupation that a person adopts for a living. A person can do a job, but there could be many reasons for adopting a particular career. What are the specific reasons that have motivated/inspired you to pursue the medical profession?
Feedback:
Average Response: I have also seen the amount of money a doctor can earn. The rewarding and social status of a doctor has inspired me to pursue and adopt this career. Excellent Response: I've grown up in a family filled with doctors. I've carefully observed them. Their attitudes towards healthcare have been impeccable. These observations have inspired me a lot throughout my life. After watching them, I tried to understand the working and reasoning behind their hard work. I saw them saving lives, putting their efforts wholeheartedly, and winning social respect and financial grants and rewards. I then directly asked the doctors of my family and reached out to many other doctors as well. Their thoughts and work inspired me. Their good financial status, along with respect earned, has motivated me to adopt this profession.
MMI Question #38

Question: During a monthly assessment of 1st-year medical students, the examiner explains that a profession requires you to be honest and hardworking towards it. Good and bad times are part of your life. One day you enjoy your profession, and the next day might be the most challenging day of your life. However, if your profession is not something you like, you might just get frustrated with the hard times in your career. What past activities have you done that show your interest in medicine?
Feedback:
Average Response: I have done tremendously well in my academic career. I have always chosen subjects and fields that are related to medicine. I have always given my best in most of them. Excellent Response: The medical profession has always intrigued me since my early childhood. I have always watched my cousins grow in medicine. It is what has inspired me a lot. I have not only done well previously in my academic medical career. I have also written many articles and blogs related to medicine. I have also been the leader of the medical society at my college. I have recently published an article about COVID vaccines on medium as well after extensive research and study.
MMI Question #39
Category: MMI Personal
Question: The field of medicine is comprehensive, and there are many different types of health caregivers involved in it. Everyone has a specific role and importance. If you want to serve as a healthcare provider, you can choose any profession related to health care provision. Why did you choose to become a doctor and not a nurse?
Feedback:
Average Response: I wanted to become a doctor because doctors are at the top of the list when providing healthcare services. So, I want to be at best possible level. Excellent Response: Everyone involved in the health care service is significant and has its value. Nurses' roles and services cannot be ruled out as they play a critical role in providing health services. However, the doctor is the best fit for the job when it comes down to knowledge and managing any critically ill patient. The importance of the role of nurses cannot be ruled out at the same time; the doctor's skills and knowledge make him a better option that has appealed to me the most. This is what has inspired me to become a doctor rather than becoming a nurse.

Question: John is a second-year medical student preparing for his end-of-year examinations. One week before his examinations, he has a Molecules, Cells, and Diseases coursework due. One of his friends has just emailed him a selection of coursework completed by senior students in prior years for the same project. Explain your actions if this would have happened to you? Do you consider this as cheating and unethics?
Feedback:
Average Response:
• I will not consider using the coursework as this will be against the ethics. If I get caught, the examiner will punish me for this.
• I will also strictly suggest my friend not to use the coursework help or get in trouble.
Excellent Response:
My priority would be to talk to my friend as early as possible to avoid any unpleasant situation. I will appreciate his intentions which are good in terms of sharing the work. However, I will explain to him why this is not a good idea and a violation of the coursework due to several reasons:
• Sharing coursework compromises the entire assessment procedure. Furthermore, it may harm our ability to become suitable medical professionals.
• By sharing the same coursework, it is possible that my other colleagues would also come up with similar work, which would likely result in disciplinary action.
• Additionally, I would want to establish if the coursework had been distributed with the consent of our seniors because plagiarism accusations would likely involve disciplinary action towards them as well. If the coursework was distributed without their permission, then I will inform them of this and advise them not to share their coursework with anyone in the future.
MMI Question #41
Category: MMI Ethics
Question: You are sitting in an examination hall for your final year medical exam. A minute before your exam starts, an older guy comes and sits right beside you. The papers are distributed, and the teacher has allowed the students to start writing. During the exam, you found the guy doing some suspicious acts. He behaves nervously, and whenever you look at him he hides his face and tries to put something in his pocket. You begin to suspect that he might be doing something wrong. How will you handle the situation?
Feedback:
Average response:
• I will inform the supervisor or the teacher in charge about the cheating
• I will confront the old guy after the exam and explain that he should not be facilitating academic cheating.
Excellent response:
• Academic dishonesty is never ethical, and it poses a potential danger to the future of both the students and patients.
• Just because I do not know the guy sitting next to me, it doesn’t mean that I should not raise the concern to the supervisor in the examination hall. I must report such events. So, instead of creating a scene in the hall and putting other students undergoing exams under stress, I will silently raise the concern after finishing the exam.
• Another way can be to reach out to the student after the exam and ask who is his lecturer was so that I can raise the concern to his supervisor directly. Creating drama during the exam will be inappropriate, especially if I am incorrect and he is innocent. So, I will have to tackle the entire situation wisely.

Question: Daniel is 70 years old and is admitted to the hospital due to kidney failure. A few hours later, Richard, 24 years old and a drug addict, is admitted for the same reason. If you are in charge and have to decide which patient will get the kidney transplant, how will you choose between both of these patients?
Feedback:
Average Response:
I will approve the kidney transplant for the 24-year-old Richard because he is young and more likely to live longer.
I will approve the kidney transplant for the 70-year-old man because the damage to the kidney is due to a disease and not lousy eating habits.
Excellent Response:
The health care system works as a team. I have to get advice from other members of this system or the transplant committee.
I have to be non-judgemental while considering who is more deserving of the kidney transplant and how much it will benefit him in his future life.
I want to match the kidneys for compatibility because there is always a risk of transplant rejection. Either of the patients can be chosen if the other one is not fit for the available kidney.
Furthermore, I will explain appropriately to the family members of the person who is not getting a transplant what\'s the reason behind it.
I will ensure the proper care and treatment of their patient while keeping him on the transplant list.
I will also assure them that our institute has followed proper guidelines while allocating organ transplants without being judgemental.
Other similar situations should be evaluated before choosing (the perfect candidate) for the particular transplant.

Question: Jenny, a 22-year-old patient with Down syndrome, has become pregnant. She does not want an abortion, but her mother and father want her to have an abortion. What should you do as the physician taking care of this patient?
Feedback:
Average Response:
I will favor the parents while keeping in view the health-related issues of the patient.
I will go for an abortion in the better interest of the patient to save her life as she is already suffering from a genetic disease.
Excellent Response:
First of all, I will keep in mind what is suitable for the patient. How much is she competent to have a baby? Furthermore, I also have to respect her parents\' autonomy.
I will check whether Jenny is competent to have a child or not.
I will talk to Jenny about why she wants to have a baby or her awareness of the responsibilities of having a baby.
Then, I will ask her parents about her competency and why they think abortion is the best option for their daughter. Did they think that pregnancy is harmful to their daughter?
Finally, I will return to the patient after inquiring about her parents separately and tell her why her parents want an abortion for her?
In the end, I will decide in the patient\'s best interest while keeping in view her competency and health status.
